1. Secure an endorsement letter from Coop Health through the provided Google Form:

2. Fill out the form completely. (Note: If submitting another request, use a different email address.)

3. Requests must be made 5 days prior to their preferred date.

4. Once approved, the endorsement letter will be sent via email.

5. The endorsement letter is valid for one (𝟏) 𝐦𝐨𝐧𝐭𝐡.

6. The APE must be conducted in our 𝐚𝐜𝐜𝐫𝐞𝐝𝐢𝐭𝐞𝐝 𝐜𝐥𝐢𝐧𝐢𝐜𝐬 only (not hospitals).

7. For laboratory tests not included in the list, please contact the Coop Assurance Center Office at 𝟎𝟗𝟔𝟕-𝟎𝟑𝟒-𝟑𝟖𝟑𝟐.
